Does using a Feliway diffuser become less effective over time?
I began using the Feliway diffuser when my female cat stopped using her litter box to urinate. It worked wonders! That said, over time and with the introduction of a puppy to the household, it has been less effective. Poor thing, she will go anywhere else but her box (the bathtub is a favorite, but the carpet is no less immune), and I'm even using Cat Attract cat litter. She's obviously stressed, but I don't know what else to do to make her more comfortable. Does anyone have any suggestions?

Your cat is feeling disenfranchised and vulnerable. Create a "cat only" nook in a secure dark place (closet with a nice bed to curl up in) or a box on it's side with an old, but smells like you, T shirt.
Put the puppy away to it's own place as well for a couple of hours a day and let it sleep and let the cat have the run of the house. They will work out their relationship, but it also depends on the age of your cat and how long it ruled the house before puppy arrived. Each deserves a space and time of their own and shared time too under watchful eyes.
Try shredded newspaper in the litter box, then add plain litter a little at the time IF she uses the paper, then switch to 60-70% plain clumping litter to balance of "smells covering" litter. Also, my litter boxes are IN the Tub (we use a shower) and small plastic dish pan is in front of the box to catch loose litter from feet. It really keeps the stray litter to a minimum. I have lots of cats and only one has a "not in this place" when under stress and finds the dish pan more to her liking or the bathroom sink, but for the most part uses the box (unless it needs a clean out-which I do 2-3 times a day). She is just persnickety.
