They give you 4 blank lines to put what you want to write on it. I myself put my dog's name at the top then my husband's cell phone with area code and his first name, then on third line my cell number with area code and my first name and then our street address just the house number and street name.
but you but what ever you want! Make sure you realize if you don't capitialize then your tag will look EXACTLY like you typed. I made sure I put my dog's names in all caps.

You don't....you automatically get a number on your tag and when you get the tag, you go to a website and fill out all your information and connect the ID # to your personal info for contact.
